Daily Screening Forms
All Faculty and Employees need to fill out the daily symptom screener. Please fill this form out within an hour of expected arrival to our campus. Everyone coming to campus must take their temperature within one hour of arriving on campus.
On Campus Work
Employees are to consult with their supervisor/department head in advance to be sure physical distancing within their office space has been confirmed and secured. -Within one hour before coming to campus, employees are required to complete the online screener, take their own temperature, and receive authorization to enter campus (once daily)
While on campus, employees and faculty must wear a mask while in common areas (a mask will be provided to those who need one); wash their hands for at least 20 seconds with soap and water after using the restroom; avoid coughing or sneezing into their hands; cover their mouth and nose with a tissue or upper sleeve when coughing or sneezing; put the used tissue directly into a wastebasket; clean their workstation with normal cleaning agents that will be provided; and have disposable wipes handy to wipe down commonly used surfaces. If soap and water are not readily available, they must use an alcohol-based hand sanitizer with at least 60% alcohol (additional hand-sanitizing stations have been placed around campus for everyone’s use); keep a 6-foot distance from others at all times; avoid hand-shaking.
Employees are encouraged to continue working remotely where possible, upon consultation with their supervisor/department head. Those telecommuting on a consistent basis at the start of the fall 2020 term will be required to complete a Telecommuting Agreement. Once authorized by supervisor/department head, the agreement will be maintained in the Human Resources Office.

Students who do not have an indicator are attending in person.
Student status will be updated throughout the term to keep track of students who change to remote attendance for part or all of the term. Once the term starts, you will be notified if someone in your course switches to online attendance mode.
You are welcome to allow your in-person students to attend remotely, if you choose, but not vice versa. Students who have chosen to be remote are not following the on-campus testing protocol and so should not come on campus; likewise, students who have tested positive or are in quarantine should not come to class. For these reasons, please do not allow students marked as “online” to attend in person without prior clearance from Health Services, the Dean of Students, or myself.
If small in-person meetings do occur indoors, everyone should wear masks and adhere to 6-foot distancing.
